Bridge Builders

Community Bridge Builders offer free one-to-one support to enable people to think about the things they would like to do and where they can be done. Defence Medical Welfare Service (DMWS) external-link.gif

The St John and Red Cross Defence Medical Welfare Service (DMWS) is a registered charity that works for the Ministry of Defence and with other charitable partners to provide practical and emotional support to military personnel, their families and other entitled civilians when they are in hospital, rehabilitation or recovery centres.

DMWS Welfare Officers support patients from all three services, wherever they serve, including working within Field Hospitals during times of conflict. If community groups and organisations know of someone who is in need and is a former services person, what should they do? Anyone can refer to DMWS.  We provide a 24/7 on-call service.  Routine and emergency referrals, plus enquiries can be made to our duty mobile on 07715 436671.  All referrals are treated with the strictest of confidence and we are always happy for you to keep identifiable details from us until you and we are confident that DMWS has a role to play.

Integrated Community Nursing Teams

This service is provided across Warrington by Bridgewater Community Healthcare NHS FT. It consists of District Nursing, Community Matrons, Treatment rooms and OOH District Nursing. Community Nursing services  provide 24 hour service, 365 days per year to deliver high quality nursing care and treatments to housebound patients in a variety of setting but mainly within the patients home, this can include those living in residential care homes, extra care housing and sheltered accommodation. The integrated nursing teams work with GPs, other specialist nurses, Social Services departments, allied health professionals, acute hospitals and all other health care providers to support and maintain and improve the health and wellbeing of the residents of Warrington and St Helens. Kooth – Free online advice for young people external-link.gif

A unique service that provides vulnerable young people, who have emotional or mental health problems, with support when they need it most. Kooth was developed to provide PCTs, local authorities and GPs with the resource, expertise and support to provide help to young people most at risk and crucially, prevent them from entering the care system. It engages with hard-to-reach young people in a way that other services can't. Kooth provides users with a free, confidential, safe and above all anonymous way to ask for help. It's currently available to over half a million 11-25 year olds in England and Wales and is now operational in 15 geographical areas. North West Boroughs Healthcare (NWBH), formerly known as 5 Boroughs Partnership NHS Foundation Trust, provides treatment, support and guidance for a wide range of health issues. These include physical and mental ill-health issues and Learning disabilities.

NWBH deliver a range of health services across a population of more than 3.5 million people. These include community-based physical health services, as well as mental health and learning disability services. NWBH also deliver community and inpatient mental health services across Warrington, Knowsley, St Helens, Warrington and Wigan, as well as services for people with mental vulnerabilities who are involved with the criminal justice system across Greater Manchester.

They also provide physical health services for children and adults in Warrington, Knowsley, Sefton and St Helens and these community-based services are now a big part of what we do. These include children’s nursing and therapy services, wellbeing services, podiatry, physiotherapy, falls prevention, end of life care and district nursing.
